BrightView Partnered with Summa Health to create the MAT Transitions of Care Toolkit

The toolkit provides guidance and documentation to help health systems develop a system of care for patients with opioid and other addictions who are hospitalized in inpatient settings. The toolkit includes protocols, practice guidelines, recommended EMR updates, and other materials to support practice changes in health systems. The toolkit is free and available online. This project was funded by Ohio Mental Health and Addiction Services (OhioMHAS) as part of the Ohio CURES STR grant.
